Gateway offers a variety of social and recreational activities that provide members with the opportunity to maintain an active and healthy social life.  Programs and activities promote independence and improves members physical and emotional well-being.

 

Gateway also provides a Community Transport service, available to members which improves access to local events and facilities

How can I join Gateway Community Services?

People aged under 65

Eligibility for Home and Community Care (HACC)

  • Are you aged under 65 and/or have a disability? 

  • or Aboriginal or Torres Strait Islander aged 50 or over?

  • Living with a chronic , physical or mental health condition

  • Are you interested in being physically and socially active? 

  • Do you reside within the cities of Hobsons Bay, Maribyrnong,

            Brimbank, Wyndham or Melton

Eligibility for Commonwealth Home Support Programs (CHSP)

  • Are you aged over 65?

  • or Aboriginal or Torres Strait Islander aged 50 or over?

  • Do you reside within the cities of Hobsons Bay, Maribyrnong, Brimbank, Wyndham or Melton

People on an Aged Care Package

If your are receiving or in the process of being assessed for Aged Care Package levels 1,2,3 or 4, we will need to determine if your current package has funds available for Social Support activities.​

A Fee For Service arrangement may need to be negotiated with Gateway, contact our office to discuss your individual needs.
