People on an Aged Care Pack Package (ACP)

 

If your are receiving or in the process of being assessed for Aged Care Package levels 1,2,3 or 4, we will need to determine if your current package has funds available for Social Support activities.​

A Fee For Service arrangement may need to be negotiated with Gateway, contact our office to discuss your individual needs.
